Insurance Timelines to Make Changes

Further details may be found in the information brochures provided to employees at new hire orientation or from the Human Resources Benefits Division. To add or remove dependents completed enrollment forms and required documentation must be forwarded to the Human Resources Benefits Division within the time frame noted below, otherwise dependents are not be eligible to be added until the next annual open enrollment period.

Additions:

  • New Eligibility—Completed enrollment forms and required dependent documentation must be received within 30 days of the eligibility or the event.
  • Marriage—completed enrollment forms and copy of marriage certificate must be received within 30 days of the marriage.  Coverage begins the date of the event.
  • Birth/Adoption—Completed enrollment forms and a copy of birth certificate (with parent’s name) or adoption papers or court order must be received within 30 days.  Coverage is effective upon the date of birth/adoption.
  • Open Enrollment—For any person not meeting the above requirements.  Completed enrollment forms must be received within the guidelines as per the related open enrollment memorandum.

Changes/Cancellations:

  • Divorce—The employee should notify the Human Resources Benefits Division in writing upon finalization of the divorce (copy of decree section pertaining to health benefits). Termination of Health Benefits is effective the end of the month in which the divorce is stamped by the Judge.  Prescription coverage terminates immediately.
  • Dependent Child (no longer eligible)—The employee should notify the Human Resources Benefits Division when the child ceases to meet the definition of an eligible dependent. Health Benefit Coverage terminates at the end of the year following ineligibility.  Prescription coverage terminates immediately.
  • Dependent Child (change of status) — The employee should notify the Human Resources Benefits Division when the child no longer requires coverage. Health Benefit Coverage terminates at the end of the month. Prescription coverage terminates immediately.
  • Separation from employment—Health Benefit Coverage terminates at the end of the month following ineligibility.  Prescription coverage terminates immediately.
